I authorize the use of appropriate anesthetics and other medications, and I understand that the veterinary technician will be emplyed as deemed necessary by the veterinarian. *

Optional authorization

I wish to authorize pre-anesthetic bloodwork for my animal. Bloodwork is highly recommended for all animals about to have an anesthetic procedure. Bloodwork can detect early signs of health problems that may affect your pets' procedure. Jr. Panel - $104.00 or Sr. Panel with Thyroid - $175.00
I wish to authorize a Pet Link microchip implant for pet identification. This implant is permanent and is good for the lifetime of the animal. The microchip is optional and is an additional $57.00

I hereby authorize any necessary tooth extractions during my animals dental cleaning. Not all dental procedures require extractions. *
